Our Care
One less thing to keep track of
Medication prompting and support at home across Newcastle, North Tyneside and North Shields, always within an agreed care plan and clearly recorded at every visit.

Support you can rely on
Remembering what to take and when can become harder, particularly when there are several medicines, changing doses or new prescriptions after a hospital stay. A regular reminder at the right time takes away a lot of worry, for the person and for the family.
Our carers work to what has been prescribed and agreed. They do not prescribe, diagnose or change medication, and anything unusual is reported to the office and to the relevant health professional.
What medication support can include
- Reminders and prompting at agreed times
- Help opening packaging and blister packs
- Support to take medication as prescribed
- Applying creams where agreed in the care plan
- Clear written records at every visit
- Keeping an eye on stock levels
- Collecting prescriptions where agreed
- Reporting concerns to the office and to health professionals
Medication support is rarely on its own. It usually sits alongside personal care, meals or companionship within the same visit.
How medication support works
- Step 01
Checking what is prescribed
We record the medication in place, the timings and who else is involved, such as the GP or pharmacy.
- Step 02
An agreed level of support
Together we agree whether support is a prompt, help to open and prepare, or assistance to take medication.
- Step 03
Trained carers, clear records
Carers complete medication training and record what was taken, or not taken, at every visit.
- Step 04
Raising concerns quickly
If something is missed, refused or looks wrong, we report it to the office and to the appropriate health professional.
Common questions
Can carers change my medication or dose?
No. Carers never prescribe, diagnose or change medication. They support what has been prescribed and agreed in your care plan, and any concerns go to your GP or pharmacy.
Can you collect my prescriptions?
Collecting prescriptions from a local pharmacy can usually be arranged as part of your visits. We will agree the arrangement with you in advance.
How is medication recorded?
Carers complete a medication record at each visit showing what support was given. Families can ask the local office team for an update at any time.
What if I refuse my medication?
That is your choice and it is respected. The carer records it and lets the office know, and we will contact the relevant health professional if it becomes a pattern.
