The first few weeks matter
Coming home from hospital is a relief, and it can also be daunting. The fridge is empty, the house is cold, there are new tablets to keep track of and everything takes longer than it used to.
Hospital to home support covers those practical gaps while your confidence returns. It is short term for many people and can simply stop, or step down gradually, once you are back on your feet.
What hospital to home support can include
- Getting the house ready before you come home
- Shopping so the cupboards are stocked
- Meals, drinks and encouragement to eat well
- Prompting and support with new medication
- Help with washing, dressing and getting ready
- Laundry, tidying and everyday household tasks
- Support to move about the home safely
- Help getting to follow up appointments
This is practical and personal support rather than clinical rehabilitation. We work alongside the health professionals involved in your recovery rather than replacing them.

Planning the discharge
We speak with you and your family, and with the professionals involved where appropriate, before you come home.

Ready for your return
Heating on, food in and the essentials to hand, so the first evening home is a calm one.

More support at the start
Visits are usually more frequent in the first weeks while things are unfamiliar and tiring.
- Step 04
Stepping back as you improve
We review regularly and reduce support as your confidence returns, or continue it if that suits you better.
Common questions
How quickly can support start?
We do our best to work to discharge dates, which often change at short notice. Contact the local office as early as you can and we will tell you honestly what we can put in place.
Is this the same as rehabilitation or reablement?
No. We provide practical and personal support at home, not clinical rehabilitation or therapy. We work alongside any therapists, district nurses or reablement teams involved in your recovery.
Can support be short term?
Yes. Many people use it for a few weeks only. There is no expectation that it continues once you no longer need it.
Can you help with new medication after discharge?
Carers can prompt and support with medication as prescribed and recorded in your care plan. They do not prescribe or change anything themselves.

Newcastle upon Tyne, North Tyneside including North Shields, South Tyneside, Gateshead, Ponteland and selected communities across Northumberland, including areas such as Ashington and Blyth.
